MPI receives complaints alleging fraud or abuse under the Florida Medicaid program using this form. You can report suspected fraud without giving your name and contact information. However, providing your contact information will help our investigation.
In order to request whistle-blower protection, you must file your complaint with the Chief Inspector General or the applicable Agency Inspector General. You can file your complaint with the AHCA Inspector General through their complaint form.
If you wish to file a complaint related to a health care practitioners’ quality of care, those complaints should be filed with their regulator, the Florida Department of Health. Use this link to access the Florida Health Care Complaint Portal.
If you are reporting a Medicaid recipient that you do not believe is eligible for Medicaid, those complaints are investigated by the Florida Department of Children and Families (DCF). Use this link to report suspected Public Assistance Fraud to DCF.
