How to file a complaint (Example)

In the top menu, click on ‘Complaints’ and a flyout menu will appear. Click on ‘File a Complaint’. (Or click here to navigate directly to the page).

  • On the first page, fill out your contact information and click ‘Next’

  • On the ‘You Can Reach Me At’ page, fill out the information in the fields shown

  • On the ‘Tell Us About Your Issue’ page, click on ‘Add Allegation’

  • On the ‘Add Allegation’ page, click ‘Add Affected Entity’ and fill out the fields as indicated. You can add more than one affected entity.

  • Fill out the rest of the fields under ‘Tell Us About Your Issue’ and click ‘Next’

  • Fill out the information related to your issue and click ‘Next’

  • On the ‘Additional Details’ page, please add a brief summary of your complaint and click ‘Next’

  • If there are any files or documents related to your allegation, please attach them and then click ‘Submit Allegation’
  • Repeat the process above if you have additional allegations. Please note or screenshot your Complaint ID if you are filing a complaint without an authenticated user.
